UnitedHealth Group (UNH) raised its full-year 2026 GAAP diluted earnings per share forecast to at least $19.50, up from $18.70 set in July, after reporting third-quarter results that showed a significant profit beat alongside in-line revenue.

The company reported fiscal Q3 2026 earnings per share of $4.69, exceeding the $4.18 expected by analysts. Revenue for the quarter matched estimates almost exactly at $113 billion, indicating the financial surprise was driven by profit rather than sales growth. Management attributed the quarter's performance to discipline and execution across the business.

Shares of UnitedHealth rose 4.5% over the two trading days through Wednesday, October 14, while the S&P 500 fell 1.1%. On Tuesday, when the results were released before the market opened, the stock slipped 0.2% as the broader market remained nearly flat. UnitedHealth moved alongside CVS Health and Cigna, which also edged lower that day, while Elevance Health rose. The stock posted its gain on Wednesday.

The improvement in profitability reflects a recovery in operating margins that began in early fiscal 2026. Through fiscal Q2 2026, UnitedHealth's operating margin returned above its level from a year earlier. The margin had declined during a year of growing sales, falling from 4.6% in fiscal Q2 2025 to 3.8% in fiscal Q3 2025 and 0.3% in fiscal Q4 2025. During that period, UnitedHealthcare, the company's largest segment, grew revenue by 15.7%, and Optum Rx grew by 16.1% in FY2025.

Margins recovered to 8.0% in fiscal Q1 2026 and 7.1% in fiscal Q2 2026. Because fiscal Q2 2026 revenue grew only 0.4%, the additional profit resulted from retaining a larger share of each dollar rather than increasing sales volume. However, on a twelve-month view, profit has not yet returned to prior levels. Diluted earnings per share were $15.54 in the twelve months to June 2026, down from $23.08 a year earlier.

Management's raised forecast for full-year 2026 remains a forward-looking estimate rather than a realized result. The next report, covering the fourth quarter, is expected on or around January 25, 2027. Investors will watch whether full-year diluted earnings per share meet or exceed the new threshold of $19.50 established by management.
