Jack Osbourne disclosed that medical professionals advised his father, Ozzy Osbourne, not to perform his farewell concert due to severe health risks, a warning issued just 17 days before the rock legend's death. Speaking on "Great Company with Jamie Laing," Jack recalled that doctors cited "energy expenditure" and multiple ongoing medical complications as reasons to cancel the show. He described the medical team's concerns as stark, noting they told Ozzy he did not have the physical capacity to complete the performance.

The Black Sabbath frontman was dealing with heart issues and arterial problems in the weeks leading up to the event. Jack stated that Ozzy experienced a significant number of medical mishaps prior to the concert. Despite these warnings, Ozzy remained determined to take the stage for one final time. His response to the medical advice was direct: "I'm doing it." Jack explained that while he is deeply heartbroken by his father's passing, he is not angry at Ozzy for choosing to perform against medical advice. He emphasized that Ozzy wanted control over how his journey ended.

Ozzy Osbourne performed his final show on July 5, 2025, at the "Back to the Beginning" farewell concert in Birmingham, England. He appeared alongside his Black Sabbath bandmates Geezer Butler, Tony Iommi, and Bill Ward. The 10-hour event was hosted by Jason Momoa and featured guest performances from Metallica, Guns N' Roses, Jack Black, and Steven Tyler. During his closing set, Ozzy was lifted onto the stage seated on a black throne decorated with skulls and a black bat. He performed songs including "Crazy Train" and "Mama I'm Coming Home" while remaining seated throughout the set.

According to Planet Rock Radio, Ozzy addressed the crowd during the performance, stating, "It's so good to be on this stage... You have no idea how I feel," and thanked the audience from the bottom of his heart. He died on July 22, 2025, at the age of 76. His death certificate listed cardiac arrest, acute myocardial infarction, coronary artery disease, and Parkinson's disease as causes.

Sharon Osbourne has also shared details about her final moments with her husband of 43 years. In an interview on "Piers Morgan Uncensored," she recalled that Ozzy woke her around 4:30 a.m. on the morning of his death. He asked her to kiss him and hug him tight, which Sharon confirmed were his last words. She noted that he had followed his usual morning routine of exercising for 20 minutes before collapsing. Sharon described hearing screaming in their home and rushing downstairs to find people attempting to resuscitate him. She told them to stop, stating she knew immediately that he was gone. They transported him by helicopter to the hospital for further attempts, but he did not survive.
